Learning Contract Learning Contract

All students who wish to register for their internship must complete and submit an Internship Learning Contract to the Internship Coordinator to obtain academic approval to register. Approval to register for your Internship will not be granted until this form has been submitted.

To be submitted by employers providing an overall assessment of the intern’s job performance.

Internship Hours Log Supervisor Evaluation

Enter hours every time you attend your internship. Keep a running total. You must do a minimum of 90 hours for your internship..

 

This information applies to students who enter this degree program during the 2011-2012 Academic Year. If you entered this degree program before the Fall 2011 semester, please refer to the academic catalog for the year you began your degree program.
